子供のくる病患者が増加傾向にある中、牛乳アレルギーの男児がビタミンDの欠乏により「くる病」と診断されるという初めての症例がこの度報告された。
ビタミンDの欠乏により骨が軟化して脆くなる「くる病」は、米国に於いて最近まで患者数がごく僅かであったため、近頃の子供たちが充分なビタミンDを摂取しているかどうかが懸念される。今回報告された症例の２歳児はカナダ北部に在住しており、やはりビタミンDが欠乏していた。生後６ヶ月の頃、この男児は牛乳を主原料とする粉ミルクなどを飲むと発疹が現れたという。そこで母親は男児に乳製品を一切与えなくなったが、乳製品に代わるビタミンD含有食品を食べさせることもしなかった。一年半後、男児は歩くことはもちろん、一人で座る姿勢をとることすら出来なくなったのである。医師は「くる病」と診断し、ビタミンDとカルシウムを投与したが、彼が再び起き上がれるまでには２週間の入院生活を要した。
米国小児科学会は、乳児だけでなく青年になるまでは、一日に少なくとも２００IUのビタミンD摂取が必要であると述べている。牛乳アレルギーの子供を抱える親に課せられるのは、認定栄養士が作成した「牛乳以外の食品によるビタミンDの摂取が可能な食事メニュー」を参考にするなどの試みだろう。

Milk Allergy Caused Rickets:
A new case of vitamin D-deficiency, which caused rickets in a boy who was allergic to cow’s milk, was recently reported, and such a vitamin D-deficiency is suspected a cause of a growing number of rickets in children.
In the U.S., rickets, a softening and weakening of the bones caused by vitamin D deficiency, was not common until recently, and there is concern whether today’s children are getting enough vitamin D. The two-year-old boy, who did not get enough vitamin D, was living in northern Canada. When the boy was 6 months old, he had a rash caused by drinking a cow’s milk formula. Therefore, his mother stopped giving him any milk products without giving him another source of vitamin D instead. A year and a half later, the boy could not walk or even pull himself to a sitting position. He was diagnosed with rickets and given vitamin D and calcium by the doctor. A two-week hospitalization was needed for him to be able to stand again.
The American Academy of Pediatrics recommends that all infants and adolescents need to have at least 200 IU of vitamin D per day; therefore, parents of children with cow’s milk allergy should follow a nutritionally complete diet prepared by a qualified nutritionist.
